Ibuprofen是常見的消炎止痛藥物，由於其止痛效果優越，故常開立用於各種況狀的疼痛緩解，但由於傳統Ibuprofen藥效發揮慢，達到血液最高濃度約需1-2小時。所以許多藥廠紛紛研發速效型的Ibuprofen，今天微笑藥師網簡單的比較這些速型Ibuprofen的差別。
目前常見的速效型Ibuprofen分為添加Lysine與Arginine兩種，微笑做個簡單的比較：
一、Ibuprofen Lysinated
由於傳統Ibuprofen acid的溶解度很差(僅<0.1%)，導致服用後1-2小時才會到達最大血液濃度Cmax，大大影響其實用性。而Ibuprofen lysinated的溶解度為17%，服用後約30-60分鐘，即可達到血液最大濃度。【1】
下圖為比較三組不同劑型的Ibuprofen的吸收狀況，分別為已完全溶解的液態膠囊、Ibuprofen lysinated錠、糖衣包裏的傳統Ibuprofen錠，其中液態軟膠囊組與Ibuprofen lysinated的Cmax值明顯高於傳統Ibuprofen；而液態軟膠囊組與Ibuprofen lysinated組的Tmax(達到最大血液濃度的時間)明顯較傳統Ibuprofen錠小，顯示Ibuprofen lysinated有較快的Onset及較高的有效血液濃度值。【2】
二、Ibuprofen (as arginine)
Ibuprofen(as arginine)，用利Arginine做成鹽基，最主要是想增加Ibuprofen的溶解度，從廠商資料顯示，Ibuprofen arginine的Tmax為30分鐘比傳統Ibuprofen劑型的Tmax為1-2小時，Ibuprofen arginine藥效快許多。而在血中濃度上方面，Ibuprofen arginine的Cmax比傳統Ibuprofen的Cmax高了約60%。
添加Arginine除了能增加Cmax值與減少Tmax值外，另一個好處是「胃黏膜保護作用」。研究指出，精胺酸之所以具有胃黏膜保護作用，可能與精胺酸能增加胃部的血液循環有關【4】。在服用後的90分鐘，精胺酸可能主要藉由cyclooxygenase/prostaglandins途徑 (COX/PGs)達到增加胃部血流，在服用後約6小時精胺酸則主要藉由抑制iNOS/NO反應來達到保護胃黏膜的作用。【3】在小鼠實驗中發現，同時服用Ibuprofen+Arginine組與只有服用Ibuprofen組相比，實驗組小鼠的胃部細胞cGMP濃度與胃黏膜的PGE2濃度均較對照組高，顯示Arginine對於胃黏膜的保護作用【3】。
有趣的是，有研究顯示，添加Arginine的可以增加Ibuprofen的抗發炎能力，推測可能是Arginine釋放的NO的關係。【5】
